Patients are welcome to come into the surgery to collect forms and paperwork, book or check-in for an appointment.
For pre-booked appointments you can either check-in at the reception desk or you can use the automatic check-in machine.
If you have a face to face appointment after you have checked-in please wait in the waiting room for the doctor or nurse to collect you.
Please note the Minor Injuries Department, Physio, Out-Patients, INR Clinic, X-ray, Dermatology and Blood Clinic are all located in Haslemere Hospital next door to the Health Centre.
